Rise Up Counseling (“Rise Up Counseling,” “we,” “us” or “our”) is a mental health practice in New Jersey with offices at 9 S Main St, Marlboro, NJ and 88 Orchard Rd, Skillman, NJ, and virtual services across New Jersey.

This Privacy Policy explains how we collect, use, share and protect information when you visit our website at riseupcounseling.net (the “Website”), submit a form, or contact us by phone, text or email before becoming a client.

This policy covers information collected through the Website and through early contact with our team, such as appointment requests and general questions.

Once you become a client, the health information we create and maintain as part of your care is protected health information under the Health Insurance Portability and Accountability Act (HIPAA).   • Appointment preferences, such as a preferred clinician, office location, appointment format (in person or telehealth), and preferred contact date and time.
  • How you heard about us, such as a search engine, referral or insurance directory.
  • Information about what you’re hoping to address, in your own words. Please share only what you feel comfortable sharing.
  • Insurance and payment information, such as your insurance plan, and, if you choose, a copy of your identification and the front and back of your insurance card.
  • Program and service details, such as interest in an intensive, a veterans assessment or medication management.
  • Anything else you send us by phone, text message or email.

  • Comply with legal, licensing, professional and record-keeping obligations, and protect the rights and safety of clients, staff and others.

Health information you share online

Our Website forms are designed for appointment requests, not for ongoing clinical communication. Please keep the details you share brief, and save sensitive clinical information for your conversation with a clinician.

If you choose to upload documents such as identification or insurance cards, we use them only to help with scheduling, verification and billing. Where our forms and systems handle health-related information, we work with service providers that agree to protect it as required by law, including HIPAA business associate agreements where applicable.

How we share information

We do not sell your personal information. We share information only as described below:

  • Service providers that help us operate the practice and the Website, such as website hosting, form and scheduling tools, practice-management and electronic health record systems, and phone, text and email services. They may use the information only to provide services to us.
  • Rise Integrative Health, when you request medication and lifestyle management, a medical evaluation or other services provided in cooperation with them, so the services you asked for can be coordinated.
  • Insurance companies, to verify benefits and submit claims, as permitted by law and your authorization.
  • Legal and safety reasons, when required by law, subpoena or court order, or when we believe disclosure is necessary to prevent serious harm to you or others.
  • Business changes, such as a merger or transfer of the practice, subject to applicable confidentiality obligations.
  • With your consent, or at your direction.

Message and data rates may apply. You can ask us at any time to stop sending text messages (or reply STOP where supported), and we will honor your request for future non-essential messages.

Standard email and text messaging are not fully secure. Please avoid sending detailed clinical information this way, and tell us if you would prefer a different method of communication.

How we protect information

We use reasonable administrative, technical and physical safeguards designed to protect the information we collect. However, no website, transmission or storage system is completely secure, and we cannot guarantee absolute security.

How long we keep information

We keep information only as long as reasonably necessary for the purposes described in this policy. Information that becomes part of a client record is retained as required by New Jersey law, professional licensing rules and HIPAA. Inquiries that do not lead to services are kept only as long as reasonably needed to respond and for legitimate business and legal purposes.

Your choices and rights

You may ask us to access, correct or delete personal information you have provided through the Website, or to stop sending you non-essential communications. Some information may need to be kept to meet legal, licensing or record-keeping requirements.
